User understands that the Company employs the Care provider and not the User
The Company will screen Care providers for criminal history, sex offender's registry, driving
records, and social security identity. The User will have the opportunity to contact such
references, and Company urges user to do so.
User acknowledges and agrees that the Company shall not control the duties, time, method,
manner, or means of accomplishing the objectives of User needs during the assignment but
retains the right to step in if the User's needs go outside of the requirements provided.
User acknowledges and agrees that the User cannot request and or make the Care provider
perform tasks outside the selected scheduled services.
User acknowledges and agrees that the Company retains the legal right to remove a Care
provider from the User's assignment if the Company assesses and determines that the User
exposed the Care provider to any form of danger or harm.
User acknowledges and agrees that ONLY the Company shall control the compensation of any
User acknowledges and agrees to save a credit or debit card on file as requested by the
Company after the first weekly invoice. User acknowledges and agrees that ALL payments for
services rendered by the Care provider will be made payable via invoices sent by MyWoosah
with credit/debit cards only. No form of CASH or DIRECT payment for services rendered shall
be made to the Care Provider for any reason whatsoever.
User acknowledges and agrees that Company sends invoices every Monday.
User acknowledges and agrees that the invoice is DUE the same day; and that the User has until
Wednesday each week to remit the invoice before it will be considered OVERDUE.
User acknowledges and agrees that the Company will charge an additional five percent (5%)
in late charges, for every week the invoice is delinquent and unpaid. If outstanding invoices
remain unpaid after thirty days (30 days), Company will send User's account to collections for
User acknowledges and agrees that it is the User's responsibility to review and approve
Care provider's work timesheet at least twenty-four hours (24 Hours) before the invoice due
User acknowledges and agrees that Company has set the Care provider's workweek to be

scheduled service if the user uses service on-demand. If user makes an immediate decision to
terminate services, user acknowledges and agrees to the Company's legal rights to charge the card
on file for the final invoice.
